Will you communicate with my physician regarding my physical therapy?
Yes. We will fax your initial evaluation to your physician. We also re-assess your condition as required by your physician and write a progress note documenting your progress. Please let us know when your follow up appointment is with your physician, and we will prepare a progress note to keep them up to date on your current status.
